I agree with his thoughts as applied to diarrheal dehydration. However, his strong advocacy of oral rehydration therapy (ORT) in diarrheal dehydration should not be interpreted as relevant to other disorders of fluid and electrolyte pathophysiology. The successful use of ORT in the third world, coincident with scientific understanding of cholera toxin effects on sodium-glucose transport in the intestine, has provided both a scientific and practical basis for practical recommendations.
