We thank Dr Kaempf for his interest in our paper, and we share his concerns that the study should not be misinterpreted. We also are pleased that Dr Kaempf agrees that more cognizance must be taken of parental desires, but stand by our conclusion that our study supports this view. Dr Kaempf's experience may be different because cultural and socioeconomic differences in different regions may result in different views. However, in both instances, parental desires were taken into consideration.
