We commend Tielsch et al1 for conducting a large community-based trial under difficult circumstances but disagree with their conclusion that skin-wiping with chlorhexidine reduced neonatal mortality among low birth weight infants.
The authors reported that the use of chlorhexidine wipes significantly lowered the all-cause mortality among low birth weight infants (Table 1). We believe this conclusion is misleading, because it was based on a non–intention-to-treat analysis of a subgroup of participants.
