The uncontrolled study by Merchant et al1 raises disturbing questions. But contrary to what the authors state in the abstract, their data does not support the current AAP recommendations that all infants who are born at <37 weeks gestation be observed for respiratory instability and secure fit in their car seats before hospital discharge. Oxygen desaturation occurred equally in the preterm and term infants, and without unrestrained controls observed for the same period of time, we dont know if the bradycardic events observed in the preterm infants were greater than expected.
