Training Residents

I read with the greatest of interest the timely article by Feigin et al regarding variations in clinical practice (Pediatrics. 1996;98:186-190). They condude that pediatric training in community settings may inadequately prepare residents for future practice in smaller communities where the acuity and intensity of Critical care practiced by general pediatricians appears to be greater than that in more populous locales. As a pediatric residency training program director and practitioner of general pediatrics and pediatric infectious diseases, I share this concern but wish to place it in perspective.
