I acknowledge the reports referenced by Drs Reynolds and Hanna.1,2 These presentations indeed raised the issue of adrenal insufficiency in ill premature infants. Given the limits of space I deleted reference to the Portland Group's abstract when I added their full publication.3 For the same reason, other references were included only as required by the text. I regret and apologize for any inadvertent discourtesy resulting from my choice of references.

We agree that it is important to recognize adrenal abnormalities in the small premature infant.
