We appreciate the thoughtful comments of Drs Gaston and Keith, and share their concerns about potential toxicity associated with new therapies, including nitric oxide (NO). As we have emphasized in several publications, the potential for NO-related toxicity should be of foremost concern to investigators using this experimental drug.1-4 Moreover, the use of inhalational NO therapy must currently be limited to the most critically ill patients, for whom complications associated with conventional therapy are well recognized.
