Dr Nichols describes a baby who was found dead in the supine position after having been placed prone. The presence of lymphocytes and milk globules in the lung suggests that this baby may have been experiencing chronic aspiration. The relationships of these findings to the baby's death and sleeping position are thought-provoking and open to several interpretations. Is Dr Nichols proposing that the chronic aspiration was occurring in the baby's usual prone position, but that death did not occur until the baby rolled supine?
