Watchko and Oski1 deserve commendation for dramatizing the bilirubin dilemma with reason and style. Their alert anonymous resident no doubt has read Lucey's startling commentary2 which calls into serious question our understanding of this problem.
This scene has value that goes well beyond the newborn's jaundiced eye. Countless variations can be imagined: phenobarbital for seizures, antihistamines for stuffy ears, CBCs and chest films for surgery. As residents, we were aware of the well-known "Rule of 3, 7, 10, or 14" which states that: "The length of treatment for any positive culture is directly proportional to who's on service this month."
