Weir and Fearnow have made an extremely important observation and have thereby contributed significantly to the emedical literature. Their proposed mechanism of injury is lucid and logical. This report represents a strong consideration regarding the use of the gluteal area for intramuscular injections in infants, even when appropriate technique is used. How safe is the injection of nonviscous, lucent materials in this area? Further studies are urgently needed to clarify this point. Finally, pediatric practitioners must make honest reports of complications resulting from intramuscular injections (such as the above letter) to allow us to gain understanding in this area.
