Should Emphasis Be on Monitoring or Diagnosis

The article by Kelly et al1 is an accurate description of the symptomatic treatment and outcome of this distressing problem. We feel that such an approach should clearly be the last resort, and it is unfortunate that a need for a specific diagnosis was not stressed.

The authors do not clearly indicate the evaluation of these patients. Some patients had episodes of transient airway obstruction with apnea. The authors also indicate that some patients had clinical evidence of seizure disorders but they do not clarify whether all of these patients had EEG findings consistent with epilepsy.
