We appreciate the opportunity to respond to the letter of Drs. Moorthy and Chesney. They raise some very legitimate questions about the optimum therapeutic approach to rare diseases such as Wegener's granulomatosis (WG). Obviously, no single institution can accumulate a sufficient number of cases of WG to have statistically significant results in various therapeutic trials, as evidenced by the fact that our six patients with WG represent the largest single experience in the literature with this disease in children1 and our institution has one of the largest experiences with this syndrome.2,3
