Disagreement on Two Points With Commentary on Hypertension

The commentary entitled "The Pediatrician and Hypertension"1 deals with a very important problem in pediatrics that has only recently come into prominence. The concluding paragraphs are particularly timely to practicing pediatricians, stressing the careful follow-up of blood pressure in children beyond the 95th percentile for age and early investigation of those with severe hypertension.

I disagree with two items in the commentary:

1. Cuff Size: In a recent editorial in the American Journal of Cardiology,2 Steinfeld et al. summarize their own experimental and clinical work and other work, indicating that the pneumatic bladder should completely encircle the limb.
