By now, with national emphasis on inpatient audit,2 it is evident that medical education and practice have neglected documentation of care giventhis despite a long-time need for accurate, concise, legible record-keeping to assist in care by multiple professionals, to support disability evaluations, to avoid malpractice claims, to effect third-party reimbursement, and to enhance clinical investigation, among others.
