The article by Darling, Fisher, and Gellis demonstrating that hiatal hernia is not an uncommon anatomic entity in infants and children in the United States as had been formerly believed is enlightening. However, one cannot help coming away with the thought that certain symptoms in infants and children are attributable to a hiatal hernia. In view of the recent advances made in the former controversy of the relationship of gastroesophageal reflux and the location of the lower esophageal sphincter (LES), the implication of this article is erroneous and misleading.
