HEPATIC HEMANGIOENDOTHELIOMA DURING INFANCY: PATHOLOGY, DIAGNOSIS AND TREATMENT WITH PREDNISONE

A hepatic hemangioendothelioma is a benign vascular tumor of infancy which usually involves the entire liver. Many infants have associated cutaneous hemangiomas and are first seen in high output congestive heart failure with massive abdominal distension and hepatomegaly. Arteriovenous shunting within the liver is confirmed by the presence of an epigastric systolic bruit and a characteristic "blush" within the liver on selective celiac axis angiography. We report a 7frac12-month-old infant with a hepatic hemangioendothelioma who improved dramatically following a 2-week course of prednisone. There was no evidence of shunting on post-treatment angiography. Other forms of therapy previously attempted, including resection, irradiation and hepatic artery ligation, are discussed.
