Immaturity, the Blood-Brain Barrier, Bilirubinemia, and Kernicterus

LESTER F. BELTER M.D.1

1 Pathologist, Richmond Memorial Hospital, Richmond 27, Virginia

Your invitation for comments and questions on the article, "Kernicterus: Revised Concepts of Pathogenesis and Management," appearing in the October, 1966 issue of Pediatrics (38:539, 1966) is accepted.

I do not question the validity of the first two sentences of the first paragraph of page 541; however, the third, "Thus, newborn and adult animals did not differ fundamentally in the functional characteristics of the blood-brain barrier for bilirubin," is misleading and probably provides little or no evidence against the concept of "immaturity" of the blood-brain barrier.
