A case of acute poststreptococcal glomerulonephritis with hypertensive encephalopathy as the only clinical manifestation is described. The diagnosis was established by isolation from the throat of a nephritogenic (type 12) group A beta hemolytic streptococci, the presence of elevated streptococcal antibody titers in the serum, a decrease in serum complement, and the characteristic renal morphology by light, fluorescent, and electron microscopy. Normal findings or minimal sporadic abnormalities were present in the urine.
